It was done by a case manager who obviously had no &lt;span class="blsp-spelling-corrected" id="SPELLING_ERROR_0"&gt;business&lt;/span&gt; in my &lt;span class="blsp-spelling-corrected" id="SPELLING_ERROR_1"&gt;oppinion&lt;/span&gt; being a case manager.  Needless to say we never got along that well.&lt;br /&gt;&lt;br /&gt;Well she wasn't doing her paperwork and I she just finally did a progress report for October of 2007.  In it she clearly falsified information which leads me to &lt;span class="blsp-spelling-corrected" id="SPELLING_ERROR_2"&gt;believe&lt;/span&gt; that she is attempting to seek revenge.  Now that I am applying for social security this is making it &lt;span class="blsp-spelling-corrected" id="SPELLING_ERROR_3"&gt;extremely&lt;/span&gt; difficult. This allows them one less way to automatically deny you - not having &lt;span class="blsp-spelling-corrected" id="SPELLING_ERROR_0"&gt;sufficient&lt;/span&gt; medical records and may even expedite the process some.&lt;br /&gt;&lt;br /&gt;Here are some things you need to know in order to obtain your medical records:&lt;br /&gt;&lt;ol&gt;&lt;li&gt;You will need &lt;span class="blsp-spelling-corrected" id="SPELLING_ERROR_1"&gt;government&lt;/span&gt; issued Photo ID (A driver's licence or a State I.D. for those who don't drive works best.)&lt;/li&gt;&lt;li&gt;Get as complete records as possible, however be aware that records at times do cost money to obtain so be sure to have some saved up and available.&lt;/li&gt;&lt;li&gt;Have letters written or typed up to the agency you are attempting to get medical records from.  Most places require a signed letter stating you want copies of your medical records.&lt;/li&gt;&lt;li&gt;Every agency has different practices for obtaining medical records, and some don't even know there practice for obtaining medical records.
