Falsifying Records is "Not A Code Violation"

If you remember a couple weeks back I complained about my case manager falsifying records and back dating forms. I received a reply from recipient rights about the issue stating that backdating progress reports 6 months and putting false information in them is "not a code protected violation".

Needless to say I'm pissed.

Disgruntled Provider Falsifies Records

Well today I received a 'Progress Report' that was just completed for October of 2007. It was done by a case manager who obviously had no business in my oppinion being a case manager. Needless to say we never got along that well.

Well she wasn't doing her paperwork and I she just finally did a progress report for October of 2007. In it she clearly falsified information which leads me to believe that she is attempting to seek revenge. Now that I am applying for social security this is making it extremely difficult. So I am extremley upset.

Tips For Obtaining Medical Records

As I have stated earlier it is often a good idea to have your own medical records when applying for social security. This allows them one less way to automatically deny you - not having sufficient medical records and may even expedite the process some.

Here are some things you need to know in order to obtain your medical records:

You will need government issued Photo ID (A driver's licence or a State I.D. for those who don't drive works best.)
Get as complete records as possible, however be aware that records at times do cost money to obtain so be sure to have some saved up and available.
Have letters written or typed up to the agency you are attempting to get medical records from. Most places require a signed letter stating you want copies of your medical records.
Every agency has different practices for obtaining medical records, and some don't even know there practice for obtaining medical records. So be patient, and have plenty of time allotted.
Don't be afraid to make phone calls requesting the status of your request after a week. Agencies are good at loosing paperwork.

Denied, Oh Just Kidding!

Well, I received a letter from the Social Security Central Operations Center saying I was denied because I have not worked enough in my history. Here's the problem, I was born with Aspergers Syndrome. There is no possible way for me to have a good work history!

Well, the following day I got a letter from a Disability Determination Specialist telling me that I need to fill out this questionnaire within the next ten days to find out if I am applicable for disability.

I'm a bit confused by this, but I'm going to go ahead and fill out the questionnaire with the help of my case manager, but also schedule an appointment with Social Security again to find out what in the world is going on with my application!

Have Your Own Records

I have read in many of places that when applying for social security obtain your own medical records and hand deliver them at your appointment. Apparently when applying for social security the Disability Determination Specialists are graded on the time it takes for them to process an application - and waiting for medical records is time.

So I plan to have my own copies of all my medical records when I walk in the door on Febuary 1st for my first appointment.

The Beginning

I have just started applying for social security, and the reason I have waited is because it sounds like such a long drawn out process - also its the feeling of loss of Pride that is involved, admitting that you will not be able to hold a job is not the easiest thing to do and something I never wanted to do.

I completed the first part of the application online and then called the local social security office to schedule my appointment. They scheduled me for the first of Febuary to sign releases for my medical records and to go over my information.

I am going to attempt to get my case manager from the Mental Health system to help me obtain copies of my records because I have read that it makes it easier if you hand them the information.
